Accuracy of Scheimpflug topography system and optical biometry for intraocular lens power calculation after penetrating keratoplasty

Abstract Details

Purpose:

To compare Scheimpflug topography and optical biometry (OB) methods in terms of refractive outcomes for appropriate intraocular lens (IOL) power calculation in patients underwent phacoemulsification and IOL implantation surgery after penetrating keratoplasty (PK).

Setting:

University of Health Sciences, Beyoglu Eye Training and Research Hospital, Department of Ophthalmology, Istanbul, Turkey

Methods:

Twenty-eight eyes of 24 patients who underwent phacoemulsification and IOL implantation surgery after PK were included in the study. IOL power was determined using the SRK-T formula of keratometry (K) values obtained from Scheimpflug topography and OB. Patients' demographic data, medical history and best corrected visual acuity were reviewed from medical records retrospectively. Intraocular lens powers calculated by K values of the both methods and their effect on final refractive outcomes were evaluated.

Results:

The mean age was 53.92±16.83 years. The mean K values of OB and Scheimpflug topography were 42.78±4.24 D, 43.51±5.19 D respectively. (p= 0.569). There was mild correlation between two methods with Bland Altman test. Reliability of measures was evaluated with the Intraclass Correlation Coefficient (ICC). ICC with 95% confidence interval of Mean Absolute Error (MAE) was 0.802 (0.552-0.913). MAE of OB was -1.24±4.09 D and MAE of Scheimpflug topography was 0.57±7.03 D (p= 0.480). Bland Altman plot comparing MAE of the optical biometry and topography showed a high correlation. There was no significant difference in the accuracy of two methods.

Conclusions:

The MAE results point out that both methods give accurate results and can be safely used in calculating IOL power for post-PK phacoemulsification surgery.
