The effectiveness and safety of Xen implantation in cases of failed trabeculectomy

Purpose:
To evaluate the effectiveness and safety of Xen implantation in cases of failed trabeculectomy.
Setting:
Retrospective review of 13 Xen surgeries performed following failed trabeculectomies in University of Health Sciences, Prof.Dr.Cemil Tascioglu Education and Research Hospital, Istanbul, Turkey.
Methods:
All cases of had careful clinical conjunctival assessment in the area of intended implantation. Unimpaired conjunctival mobility was required for implant surgery. All procedures were performed by same ophthalmic surgeon. All cases received also intraoperative subconjunctival application of a 0.1 ml of 0.2mg/ml Mitomycin C before Xen implantation.
Results:
IOP reduced from 23.4 (±1.3) preoperatively to 15.6 (±2.1) at Month 12 (p<0.05). Medications reduced from 2.6 (±0.4) preop to 1.2 (±0.9) at Month 12 (p<0.05). 3 cases had an IOP <6mmHg without choroidal effusions or maculopathy, which typically resolved by 3 days. 4 of the 13 cases required either needling. Implant failure managed with Ahmed glaucoma valve implantation due to uncontrolled IOPs occurred in 3 cases.
Conclusions:
In cases of failed trabeculectomy, the Xen stent may be a viable option as it can a safer and less invasive procedure compare the other surgeries. However, careful preoperative assessment and bleb management is required for Xen to achieve target IOP outcomes.
