Abstract Details

Purpose:

To evaluate the safety and effectiveness of a second generation trabecular micro-bypass stent (i-Stent Inject) combined with cataract surgery in patients with mild to moderate glaucoma.

Setting:

I-Stent inject implantation combined with cataract surgery were performed at the James Paget Hospital, England.

Methods:

This study is a retrospective evaluation of 19 eyes of 19 patients who underwent trabecular i-Stent Inject implantation combined with phacoemulsification. Fifteen eyes were diagnosed with primary open angle glaucoma and one each with pseudoexfoliative, pigment, steroid, and chronic angle closure glaucoma. The primary outcome measure was the intraocular pressure (IOP), success being defined as both an IOP less than 18 mmHg and 20% reduction from preoperative baseline one year after surgery. Safety was assessed by assessing the incidence of complications at routinely scheduled exams.

Results:

The mean preoperative IOP was 21.9 mmHg. Post-operatively, the mean IOP had reduced to 12.5 mmHg at 6 months, reaching 11 mmHg at 12 months (P less than 0.005). The number of ocular hypotensive medications remained unchanged. The success rate was 100%, with all patients displaying a significant reduction of IOP. There were no cases of hyphema, hypotony, endophthalmitis, or choroidal detachment. One eye developed cystoid macular edema, which resolved by week four.

Conclusions:

From our clinical experience trabecular i-Stent Inject implantation combined with cataract surgery appears to be effective at reducing IOP whilst carrying an exceptional safety profile.
