Evaluation of the Effectiveness of Tranespithelial Corneal Collagen Crosslinking on High Order Aberrations and Optical Quality Parameters in Progressive Keratoconus

Abstract Details

Purpose:

To investigate the effect of transepithelial corneal collagen cross-linking (transepithelial CXL) procedure on visual quality and keratometric indices in progressive keratoconus patients after 12 months of follow-up.

Setting:

Hundred and ten eyes of 67 patients who underwent tranepithelial CXLprotocol (30 minutes, 3 mW / cm2) between 2014 and 2019 years were retrospectively analyzed.

Methods:

The changes in high-order aberrations (total amount, anterior and posterior values of of higher order aberrations [HOAs], vertical coma, horizontal coma, vertical trefoil, oblique trefoil, and spherical aberration), Point spread function (PSF), refractive indices, visual acuity, keratometric values measured at 4 mm pupil diameter at preoperative visit and after 12 months of treatment were compared with paired t test. The relationship between the change in corrected (CDVA) and uncorrected (UDVA) distance visual acuity with the change in parameters that have a statistically significant difference at the end of the 12th month was studied with Pearson correlation analysis

Results:

There were statistically significant decrease in total HOA (p=0.003), total coma (p=0.006), anterior HOA (p=0.07), anterior coma (p=0.012), posterior spherical aberration (p= 0.023) and vertical coma ( p= 0.05) values, while there was no statistical significant change in the mean PSF value after 12 month of CXL treatment (p=0.525). Among the topographic indices, the change in Baiocchi Calossi Versaci front index (BCVf) (p = 0.023), curvature asymetry (CAf) (p = 0.012), Root mean square (RMS)f / A (p = 0.044), and total corneal volume (p = 0.043) were statistically significant.

Conclusions:

Transepithelial CXL treatment in progressive keratoconus provides stability in keratometric indices, has a healing effect on aberrations and does not create a significant change on PSF value.
