Phototherapeutic Keratectomy in Granular Corneal Dystrophy Type 1: Four-year results

Purpose:
To evaluate the efficacy and safety of phototherapeutic keratectomy (PTK) for the treatment of granular corneal dystrophy type 1 (GCD1) in a long follow-up period.
Setting:
University of Health Sciences, Beyoglu Eye Training and Research Hospital, Istanbul, Turkey
Methods:
Forty-two patients who underwent PTK for GCD1 were evaluated retrospectively. Patients with a follow-up period of more than 4 years were included in the study. Preoperative and postoperative fourth year spheric equivalent (SE) of manifest refraction values, uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA) values were recorded. Treatment parameters which are ablation depth, optical zone, mitomycin- C application time, and complications during and after the treatment were recorded. Patients who had a decrease of two or more lines in their CDVA and those with recurrent or increased corneal opacities were considered to have severe recurrences.
Results:
Thirty-one eyes of 21 patients were included in the study. The preoperative mean SE was -0.39 ± 2.86 diopters and the postoperative SE was 0.61 ± 3.48 diopters at the end of the follow-up period. Preoperative and postoperative fourth year UDVA was 0.72 ± 0.22 logMAR and 0.30 ± 0.26 logMAR (p<0.001), respectively. The mean CDVA showed statistically significant improvement at postoperative 48th month (p < 0.05). Recurrence was detected in three eyes during the 4-year follow-up.
Conclusions:
PTK is a safe and efficient treatment method in GCD1 with a low recurrence rate in long-term clinical follow-up.
