Traumatic aniridia management – a case series

Purpose:

Aniridia corresponds to the partial or complete loss of iris tissue and can be congenital or acquired (mostly traumatic). The authors describe the management and long-term visual outcomes of traumatic aniridia corrected with intraocular lens (IOL) with artificial iris implantation.

Setting:

Case series.

Methods:

Retrospective report of 3 eyes, from 3 patients, who suffered from ocular trauma with total or partial aniridia. IOLs implanted were the ANI IOL (model 311), in 2 eyes, and a customized Reper IOL (model C) in 1 eye. All surgeries were performed by the same surgeon, at Hospital Pedro Hispano, Portugal. Best corrected visual acuity, surgical complications, subjective visual complaints and cosmetic results were evaluated.

Results:

Case number 1 represents a case of post-traumatic aphakia and aniridia. Case number 2 represents a case of scleral rupture from blunt trauma, with IOL extrusion and aniridia. Intraocular lens ANI 311 was implanted in both eyes, with scleral suspension. Case number 3 corresponds to a total cataract with aniridia and vitreous hemorrhage, following an open globe injury. Reper IOL was introduced in the sulcus, with fixation of one haptic to the sclera. All eyes improved best corrected visual acuity, with no relevant intraoperative or postoperative complications to report. A satisfactory cosmetic result was obtained in all eyes.

Conclusions:

Ocular reconstruction after trauma is challenging and aniridia can lead to significant visual disturbances, including glare, photophobia and increased optical aberrations. Implantation of intraocular lens with artificial iris allows the correction of aniridia and improves the patients’ related quality of vision. The Reper IOL is a recent and promising surgical solution for the management of aphakia and/or aniridia, with good functional and cosmetic results.
