Outcomes of scleral-fixated intraocular lens implantation with knotless transscleral Z-suture technique

Abstract Details

Purpose:

To evaluate the visual outcomes and complications in scleral-fixation of posterior chamber intraocular lenses (IOL) with the knotless Z-suture technique.

Setting:

University of Health Sciences, Prof.Dr.Cemil Tascioglu Education and Research Hospital, Istanbul, TURKEY.

Methods:

The records of patients who underwent implantation of scleral-fixated IOL with the knotless Z-suture technique in our clinic between the years of 2016 and 2020 were retrospectively reviewed and the patients with a minimum of 1-year follow-up were included in this study. The clinical characteristics, pre- and post-operative best corrected distance visual acuities (BCVA), full biomicroscopic examination results, and postoperative complications were recorded and analyzed.

Results:

Forty-five eyes of 44 patients (35 male, 9 female) with the mean age of 56.2±18.9 years and the mean follow-up of 38.3±18.1 months were included. Twenty-two (48.9%) cases had a history of complicated cataract surgery, while 12 (26.7%) cases had traumatic aphakia, and 11 (24.4%) had pseudoexfoliation syndrome. The mean BCVA improved from 0.69±0.38 to 0.44±0.34 logMAR postoperatively, and the difference was statistically significant (p<0.001). Postoperative complications included cystoid macular edema in 1 (13.3%), papillary edema in 1 (13.3%), hypotonia in 1 (13.3%) eye, and temporary increased intraocular pressure in the early postoperative period in 4 (8.9%) eyes.

Conclusions:

Our results demonstrate that scleral-fixation of IOL with knotless Z-suture technique is an effective and safe method for the treatment of aphakia.
