Phacoemulsification in soft Cataract- Bari’s Shell Technique

Abstract Details

Purpose:

To describe a risk-free phacoemulsification technique in soft cataracts.

Setting:

At three eye hospitals of Bangladesh, surgeries were performed by the inventing as well as the presenting author applying this technique during the period of November 2016 to July 2019.

Methods:

After Capsulorhexis anterior cortex and epinuclus were removed circumferentially along the rhexis margin at low power (25-30%) and vacuum(80mm) setting. Then hydro-delineation was done by pushing fluid at the junction of nucleus and epinucleus at different sites 90 to 180 degree apart. Thus cortico-epinuclear shell was created below the core nucleus. Then hydro-dissection was performed and popping up of the nuclear edge was visible. The core nucleus was caught by phaco tip at high vacuum(350mm) and low power(25 to 30%) setting, it came out of the shell with a tumbling movement, and was emulsified directly. Results:

Most of the patients, 82% (n=205) were less than 55 years of age( 21 to 54 years) and 18% (n=45) were above. Regarding gender 64%(n=160) were male and 36% (n=90) were female. No posterior capsular tear occurred during phacoemulsification of nucleus (0%) and there was no nucleus or cortex drop in the vitreous(0%). Posterior chamber Intraocular lens was given in 100% of the cases, in bag 99.6%(n= 249) and in sulcus 0.4% (n=1). One PCT (0.4%) occured during polishing of posterior capsule and one small zonnular dehiscence after IOL implantation. No additional cost was required.

Conclusions:

Bari’s Shell technique phaco is a safe, easy and effective method in soft cataract without any additional cost.
