Purpose:
To evaluate the possibility and efficacy of Corneal Refractive Therapy by Paragon CRT Lenses in myopia control in teenager population and to analyze the success and failed cases.
Setting:
: AILAS Medical Center, Kiev, Ukraine
Methods:
Retrospective analysis of 2 groups of myopic patients matching in age, sex and degree of myopia and followed up for 4 years. 1245 – were corrected by CRT (CRT group) and 144 – were using glasses or Soft Contact Lenses (Control group). Age from 8 to 18 years: mean 14.4±2.6 for CRT and14.6±3.0 for control. Axial/Mixed/Refractive Myopia in CRT and control: 606/404/235 and 71/45/28 patients. Standard investigations including refraction (for CRT patients – refraction over lens (ROL)), U/S biometry and physical efficiency of ciliary muscle (accommodative reserves in D) (ARD) were performed every 6 months.
Results:
In control group progression of myopia for 1.0D and more (per year) were in 128 cases (88.9%) – associated with increasing of axial length (AL). In CRT group 348 (27.8%) patients had progression of myopia and AL, in 899 (72.2%) of CRT patient had stable ROL and AL. The baseline ARD was 2.7+1.8D and 3.0±2.1D for CRT and control. For the CRT patients with stabilized myopia and AL ARD increased from 2.7+1.8D up to 6.9+2.7D. For the patients with progression of myopia (from both groups) it remained low: 3.5+2.0 and 3.1±2.3D CRT with progression and control.
Conclusions:
In case of stable corrective effect of CRT it can have a preventive effect too. Refractive myopia is controlled easier. Preventive effect (myopia control in teens) is associated with significant increasing of physical efficiency of ciliary muscle and can be explained by it.
