Purpose:
The aim was to reconstruct the anterior segment in a patient after complicated extracapsular cataract removal (ECCE).
Setting:
: The aphakic patient presented 200° iridodialysis with an irregular, decentrated pupil and unknown condition of the anterior and posterior capsule. The posterior segment of the eye was normal.
Methods:
The root of the iris was sutured to the sclera on four spots 40° apart using 10/0 nylon sutures on straight needles. Anterior synechiae at limbus and posterior synechiae between iris and capsule were removed. The foldable PCIOL was inserted into sulcus.
Results:
After synechiae removal, the capsulorrhexis in the anterior capsule and intact posterior capsule with no visible zonular deficit was revealed. The anterior and posterior capsules were adherent. The sufficient support enabled the IOL implantation in the sulcus. Eventually, iris was sutured to its usual location, the pupil location and shape were restored and IOL was implanted.
Conclusions:
We present a simple and efficient method to restore anterior segment anatomy in patients with iridodialysis and sufficient capsular support.
