Non-absorbable biological drainage xenoplast in advanced glaucoma with perineural scleroplasty

Purpose:

To evaluate advantages and clinical outcomes of non-absorbable collagen type I Xenoplast drainage in antiglaucomatous operations with perineural scleroplasty to achieve ON excavation reduction (3 years of follow-up).

Setting:

: Eye Center “East Sight Recover“, Moscow, Russia.

Methods:

This study includes the results of 98 cases of advanced glaucoma that were follow-up during 3 years after NPDS, or trabeculectomy with Xenoplast – drainage implantation and combination with scleroplasty to reduce tension around the optic nerve head. Xenoplast is produced from porcine collagen type I by Russian company “Transcontact” and has Russian and Ukraine registration. Clinical outcomes were evaluated in terms of IOP level, quantity of antiglaucoma medications, OCT of the optic nerve, VF (every 6 months of follow-up).

Results:

In all cases in the early post-op period (1-6 months) we received IOP normalization 17.5±1.4 mm Hg on one medication a day, in 24% of cases IAG-laser goniopuncture was performed after 2-12 months after surgery. In 3 years, IOP 17.9+2.5 mm Hg on 1-2 medications a day were received in 87% of cases. Every 6 months of OCT examination during the 3 years of follow-up, we observed reduction of the excavation volume in 63.6% of cases.

Conclusions:

Biological drainage Xenoplast provides safety in early and far follow-up. Perineural scleroplasty creates possibilities to reduce tension of sclera around the optic nerve, that helps to reduce the excavation and VF stabilization.
