Purpose:
To investigate the visual outcomes of cataract surgery in stable keratoconus patients. To evaluate efficacy, security and stability of this technique. To discuss the suitability and necessity of this surgery according to the preoperative degree of advance of the keratoconus and the cataract
Setting:
: Vissum Corporation, Alicante, Spain
Methods:
15 patients (the largest series studied until now), (6 women, 3 men) diagnosed of stable keratoconus aged from 34 to 73 years (56.63±12.47) underwent cataract surgery. (6 bilaterally, 3 unilaterally).12 Acrysoft toric and 3 Kelman were implanted. A complete ophthalmologic examination was performed pre and postoperative. The most interesting variables measured for the purpose of this study were: manifest refraction, UDVA (uncorrected), CDVA (corrected) distance visual acuities (logMAR). Follow up time was 4.10±5.54 months (1 to 15)
Results:
Sphere changed from -1.77±6.57, to 0.08±0.79 (P=0.211), cylinder from -2.95±1.71 to -1.40±1.13 (P=0.016). UDVA change from1.33±0.95 to 0.18±0.32 (P=0.008) and CDVA from 0.32±0.45 to 0.20±0.36 (P=0.013). Efficacy and safety indexes were 1.38±0.58 and 1.17±0.66, respectively. None of the patients suffered loss of lines in CDVA in Snellen scale or significant refractive changes afterwards
Conclusions:
Limitation in visual quality due to the cataract can make it totally mandatory to perform this kind of intervention. Moreover, significant improvement in UDVA and CDVA were achieved after this surgery and good results for safety and efficacy were obtained making this surgery as a secure procedure
