Pattern of irregular astigmatism and higher order aberrations in post perforating-keratoplasty eyes with femtosecond laser-assisted lamellar keratotomy

Purpose:

To investigate whether femtosecond laser-assisted lamellar keratotomies decrease corneal astigmatism and irregularities and higher order aberrations.

Setting:

: Swiss Eye Research Foundation, Eye Clinic ORASIS, Reinach AG, Switzerland and VIDAR-ORASIS Swiss, Novi Sad, Serbia.

Methods:

7 eyes that had undergone penetrating keratoplasty and showing high astigmatism, corneal irregularities and substantial higher order aberrations were treated. All eyes underwent lamellar keratotomies with an LDV femtosecond laser. Ophthalmological examination included best-corrected visual acuity, corneal topography and wavefront measurements and was performed before and at 6 weeks after treatment.

Results:

Reduction of astigmatism was not significant and the axis of astigmatism remained unchanged. We observed a significant decrease of corneal irregularities in the central 5mm zone and a highly significant decrease of aberrations of higher order. These changes induced a distinct increase in best-corrected visual acuity.

Conclusions:

Corneal grafts seem to show significant amounts of tissue tension. A lamellar keratotomy leads to tissue relaxation with a decrease in corneal irregularities and higher order aberrations.
