Purpose:
to evaluate the refractive and keratometric effect of the arcuate keratotomy (AK) performed with Femtosecond Laser in patients with high postkeratoplasty astigmatism

Methods:
14 eyes of 14 patients that had undergone a penetrating keratoplasty were considered.The subjective refraction was measured, the corneal thickness and the keratometric parameters were calculate.The incisions were performed with the femtosecond laser. Mean change in Best Corrected Visual Acuity (BCVA),refractive and keratometric astigmatism and spherical equivalent (SE) were evaluated. Refractive and keratometric data were analyzed by using vector analysis as described by Alpins. A paired test ANOVA was used to compare preoperative and 36 months postoperative data
Results:
The mean preoperative BCVA was 20/30, increasing to 20/25 postoperatively(p>0.05).The mean refractive astigmatism decreased of 6 D(p<0.05),whereas the mean keratometric value decreased of 4,6 D (p<0.05).The mean SE value did not change significantly.The surgical vectors in the refractive and keratometric analysis were calculated, showing a good predictability
Conclusions:
AK performed with femtosecond laser could be an efficacy,safe and relatively predictable treatment of high postkeratoplasty astigmatism
