The comparison of surgically induced astigmatism after temporal coaxial microincision phacoemulsification with different steep axis

Purpose:

To compare the surgically induced astigmatism (SIA) after phacoemulsification with temporal-coaxial microincision (2.2 mm) in eyes with different located steep axis.

Setting:

: Ünye State Hospital, Ordu, Turkey.

Methods:

This prospective, randomized study comprised three groups each with 20 eyes with age-related cataracts and underwent 2.2 mm temporal clear corneal incision. The three groups of the patients were divided by the location of the steep axis. The steep axes on both sides of vertical meridian (90-270 degrees line) were grouped based on the distance to the incision meridian (0-180 degree line). The steep axis was between 0-30, and 151-180 degrees in group 1, 31-60, and 121-150 degrees in group 2, 61-120 degrees in group 3. Outcome measures were including the changes in mean total astigmatism, SIA, axis deviation, uncorrected distance visual acuity (UDVA), and corrected distance visual acuity (CDVA). Astigmatism was measured by manuel keratometry readings before surgery and day-1, week-1, week-4, week-8, week-12 postoperatively. The SIA was calculated by the vector analysis using the Holladay-Cravy-Koch method.

Results:

The magnitude of mean total astigmatism was lowest in group 1 and highest in group 3 at week-12. Surgically induced astigmatism was 0,12 diopter (D), 0.21 D, and 0.29 D in group 1, group 2, and group 3 respectively. The change in the astigmatic axis deviation was highest in group 1 (18.5 ± 10.6), and lowest in group 3 (6.8 ± 5.9) (P ?.05). The mean UDVA and CDVA were not significant pre and postoperatively between groups (P >.05). Axis deviation and SIA were stable after the week-4.

Conclusions:

Based on the study, the distance between temporal incision and steep axis has a notable effect on SIA.
