The comparison of passive irrigation and irrigation/aspiration systems on the removal of viscoelastics

Purpose:

To compare the passive irrigation and irrigation/aspiration (I/A) systems on phacoemulsification results

Setting:

: Ünye State Hospital, Ordu, Turkey.

Methods:

This prospective, randomized study comprised two groups each with 25 eyes with age-related cataracts and underwent 2.2 mm temporal clear corneal incision (CCI). 1.4% cohesive viscoelastic material was used for hydrophobic acrylic intraocular lens implantation. In group 1 (passive irrigation group), the viscoelastic material was removed by using 25 gauge canula and balanced salt solution from the 2.2 mm main incision. In group 2 (I/A group), the viscoelastic material was removed by classic bimanual I/A system. The anterior chamber reaction, central corneal thickness, intraocular pressure, and visual acuity were evaluated preoperatively and day-1, day-3, week-1, week-4 postoperatively. Anterior chamber reaction was graded from 0 to 4 by using slit lamb biomicroscopy.

Results:

In group 1, there was +1 anterior chamber reaction in 4 eyes ( 16%) at day-1 examination. Intraocular pressure was increased in 3 eyes (23.4± 3.3 mmHg) and antiglaucomatous treatment was added. At day-3 examination, the antiglaucomatous treatment was stopped and anterior chamber was quiet. In group 2 anterior chamber reaction was not seen during follow-up period. The change in central corneal thickness and visual acuity were not significant between groups ( P› .05).

Conclusions:

Passive irrigation is an effective method as well as standard bimanual I/A system for removal of viscoelastics except the first day.
