Purpose:

To evaluate the clinical outcomes and patient satisfaction after simultaneous implantable collamer lens (ICL) removal and phacoemulsification with monofocal intraocular lens (IOL) implantation in ICL-implanted eyes with developing marked anterior subcapsular cataract formation.

Setting:

: Department of Ophthalmology, Medical and Health Science Centre, University of Debrecen, Debrecen, Hungary

Methods:

We retrospectively reviewed the clinical data of three eyes of two patients who developed manifest cataract prior to posterior chamber ICL implantation due to correct high myopia. One patient also developed bilateral secunder glaucoma, and the other underwent scleroplastica and photorefractive keratectomy (PRK) as well. All cataract surgeries were uneventful. Visual acuity, manifest refraction, endothelial cell density (ECD), and subjective satisfaction were determined preoperatively and 6 months after surgery.

Results:

Explantation of ICL, cataract surgery by phacoemulsification and posterior chamber IOL implantation were carried out through the same 2.6 mm clear corneal incision and the intra- and postoperative period was uneventful. Best spectacle-corrected visual acuity was improved from 0.07±0.04 to 0.17±0.08. The ECD fell from 2435±265 cells/mm2 to 2252±275 cells/mm2. No vision-threatening complications were absent through the follow-up period.

Conclusions:

Simultaneous ICL removal and phacoemulsification with monofocal posterior chamber IOL implantation is safe and meets the patient satisfaction.
