Purpose:
To evaluate the efficacy of toric intraocular lens (IOL) implantation for correcting irregular astigmatism in eyes with ectatic corneas, who undergo lens surgery.
Setting:
: 1. Ophthalmos Research and Therapeutic Institute, Glyphada Athens, Greece
2. Hygeia Hospital, Athens, Greece.
Methods:
We had three observation cases, first one eye of a 68 year old male with penetrating keratoplasty for keratoconus, 25 years ago. This patient had regression of ectasia over the corneal graft.
The second case refers to an eye of a 32 years old male with pellucid marginal degeneration who had corneal cross linking 3 months prior to lens surgery.
The third case was an eye of a 49 years old male with keratoconus.
All eyes had cataractic opacities. They underwent cataract surgery with toric IOL implantation, the first two with Acrysof Toric IOL-SN60T9 and the third with Acrysof Toric IOL-SN60T5.
In the second eye we performed additional opposite corneal incisions for anticipated residual astigmatism.
Results:
All eyes had good postoperative recovery. In all cases, the preoperative BCVA was under 20/50 with over 6 diopters Cyl correction, and all cases post operatively had BCVA of 20/25 with under 2 diopters Cyl correction one month after surgery. The refraction was stable until their last follow up visit.
Conclusions:
Toric IOL in selective cases of eyes with ectatic corneas can be a very good and sometimes spectacular solution, despite the initial manufacturer’s recommendation for use only in eyes with regular astigmatism.
