Purpose: 
To evaluate clinical outcomes and patient satisfaction in prepresbyopic patients who had bilateral refractive lens exchange with implantation of a multifocal intraocular lens. 
Setting: 
: Private clinic. Málaga, Spain. 
Methods: 
 Retrospective data of hyperopic patients under 45 years of age, having bilateral implantation of a multifocal lens to obtain refractive correction, were analyzed. Distance, intermediate and near visual acuities, manifest refraction and patient satisfaction were evaluated pre and postoperatively, with a follow up of at least 6 months. 
Results: 
 The study comprised 48 eyes of 24 patients with mean age 40 years (37 to 44 years), and implantation of one model of diffractive lens, Acri.LISA  366D (Carl Zeiss Meditec). The mean spherical equivalent changed from +6,11 diopters (D) +/- 1,98 D preoperatively to -0.02 D+/- 0.39 D postoperatively (9 patients had laser retreatment for residual refractive error). The mean binocular uncorrected visual acuities were +0.04 +/- 0.04 log MAR for distance, Jaeger 6 (J) for intermediate (60 cms) and J2 for near. 87,5% of patients were completely independent of spectacles and moderate night vision disturbances were reported in 20% of patients. Patient satisfaction was high in all cases and 100% recommended the surgery. 
Conclusions: 
 In prepresbyopic patients with moderate or high hyperopic refraction, the lens exchange for a multifocal lens provided excellent distance, intermediate and near visual acuity without spectacles. Retreatment with laser was necessary in some patients to achieve better refractive results. The patient satisfaction was high, with well tolerated night vision disturbances. FINANCIAL INTEREST: NONE
