Topical mitomycin C in the treatment of LASIK Bowman strip

Abstract Details

Purpose:

To analyze the feasibility of treatment with topical 0.02% Mitomycin C (MMC) following excimer laser photoablation before flap repositioning when Bowman strip (incomplete buttonhole) is encountered during routine LASIK.

Setting:

: Eye Clinic “Svjetlost” Banja Luka, Bosnia-Herzegovina.

Methods:

The photoablative procedure was continued after the strip became manifest and after completion some cases were randomly selected for 20 seconds of topical 0.02% MMC, other cases received no treatment before flap repositioning. Follow up were at one week, as well as 1 and 3 months.

Results:

Incomplete buttonhole flap complications were encountered in 9 cases of 2.988 LASIK procedures between October 2008 and August 2012. At three months post-op, UCVA & BCVA were similar, efficacy and safety indices were 1.1, and complications were not encountered in all MMC group (n=6). Without MMC (n=3), residual refractive error ≥0,50D, efficacy and safety indices were <1.0, corneal haze was encountered, up to 3 lines of BCVA were lost and one case developed secondary glaucoma.

Conclusions:

Incidence of Bowman strip was 0.30% in our series similar and confirming previous reports. Preliminary results suggest MMC application when the strip complication is encountered leads to satisfactory refractive results. A more extensive masked study is required to support this assertion.
