Purpose:
To evaluate predictability of corneal flap thickness with a Schwind Carriazo pendular microkeratome (110 microns head).

Methods:
Sixty eyes underwent laser in situ keratomileusis (LASIK) with a nasal hinge flap using the 110-micron head of the SCHWIND Carriazo-Pendular microkeratome. Right eyes were treated first. Anterior optical coherence tomography imaging was used for central and peripheral flap thickness measurement, Scheimpflug tomography for keratometry and pachymetry measurement and anterior segment camera for flap diameter and hinge size measurement. Evaluation included uniformity and shape of the flaps, flap thickness and central corneal thickness (CCT) correlation, keratometry and flap thickness correlation. All patients were examined preoperatively followed by examinations 1 day, one week, one and three months after surgery.
Results:
Mean flap thickness was 111,8 ± 6,4 μm (SD) in right eyes and 105,2 ± 5,9 μm in left eyes. Flap uniformity was within 5 microns. Flap thickness was significantly correlated with preoperative CCT but not with K values. Right eyes had thicker flaps than left eyes. Mean achieved flap diameter was 9,36±0,49 mm, hinge size was 4,47±0,52 mm. Early postoperative complications were immediately resolved, late postoperative complications have not been found.
Conclusions:
Flap-thickness predictability was influenced by preoperative CCT. Second surgical eyes had thinner flaps. SCHWIND Carriazo-Pendular microkeratome with the 110-micron head provided greater accuracy in intended creation of thin flaps. FINANCIAL INTEREST: NONE
