Use of toric IOL in keratoconus treatment

Purpose:

To present a case of difficult toric-IOL calculation and present possible solutions for solving high irregular corneal astigmatism in keratoconus.

Setting:

: Eye clinic University clinical center Tuzla, Bosnia and Herzegovina

Methods:

46-year-old man with cataract and keratoconus in the both eyes presented to. Preoperative examination shoved: uncorrected visual acuity (UCVA) 0.01, characteristic keratoconus corneal topography with K = 49.6 D (K1 = 45,79 D, K2 = 53,32 D) and KISA% = 301,2% and completely white cataract. Patient underwent phacoemulsification with a toric-IOL (Acrysof SN60T9; cylinder power: 6.0D) implantation.

Results:

Postoperative follow-up was 2 years while corneal topography was unchanged. UCVA improved from to 0,8. The refractive cylinder decreased from -7,3 to -1.0 D.

Conclusions:

Cataract extraction with toric-IOL implantation can be used to correct irregular astigmatism and can significantly improve visual functioning in patients with mild to moderate amounts of stable keratoconus and cataract. FINANCIAL INTEREST: NONE
