Purpose:
To compare rotational stablity of two hydrophobic IOL with different haptic design (Z-Flex 860FAB and Bi-Flex 877FAB, Medicontur, Hungary) during the first three postoperative months.

Methods:
Prospective single center single, surgeon study was performed. After uncomplicated phacoemulsification IOL was implanted in the bag and photos were taken in retroillumination at day 1, week 1, month 1 and 3. Images were analyzed with the help of photshop to compare the rotational stability during this period.
Results:
No clnically signficant diffennce between both haptic designs were found. Both IOL types performed well and 24/29 eyes in 860FAB group and 32/38 eyes in 877FAB group showed minimal rotation with maximum of 1°. No IOL showed more then 5° of rotation.
Conclusions:
Both hdrophylic acrylic IOL design showed excellent postoperative rotational stability between day 1 and month 3. Intraperative misallignment of toric IOL seems to be the major cause for unsatisfactory refractory results.
