Purpose:
To attempt to prove the efficacy of combined Phaco procedure and ECCE during cataract extraction in one eyed patients having pseudoexfoliation, floopy iris syndrome and very dense cataract
Setting:
: Eye Clinic, UHC "" Mother Theresa"" Tirana, Albania
Methods:
20 eyes from 20 one eyed patients, over 75 years old,having very dense nuclear cataract , pseudoexfoliation and floppy iris syndrome underwent cataract surgery.
10 patients of the study group underwent phaco technique converted to ECCE, with sphincterotomy,with or without capsular ring, with or without suturing of the iris.
10 patients of study group underwent phaco surgery , with sphincterotomy or iris hook, in the bag or in the AC emulsification, with or without capsular ring.
Bag integrity and stability, pupilar deformity and corneal decompensation were evaluated in the two groups.
Success rate was considered :
Presevation of bag integrity with or without capsular ring,Preservation of sphincter function with or without iris hook or small sphincterotomies, with or without suturing of the pupil.
Clear cornea three weeks postoperatively.
Results:
Complete success rates related to bag integrity control were:
100% of the study group had bag preservation with 40% of these cases having the help of a capsular ring.
60% of the the control group had bag preservation with the help of the capsular ring
Complete success rates related to preservation of sphincter function control were:
100% of the study group had preservation to sphincter function with 0% use of iris hook and 40% of cases with iris suturing procedure to reform the pupil.
50% of the control group had preservation to sphincter function and the other 50% lost of integrity due to use of iris hooks.
Complete success rates related to corneal damage control three weeks post op were:
100% of the study group had clear cornea three weeks post op.
60% of the control group had corneal damage due to AC emulsification or high post op IOP, 10% mild edema and only 30% had almost no edema.
Conclusions:
The study found that combination of Phaco with ECCE technique in one eyed patients having pseudoexfoliation, floppy iris syndrome and dense nuclear cataract give very good results related to capsular, sphincter funcionality and corneal integrity preservation.
This approach is associated with better final visual outcomes and lesser complication rates.
The small sample size should be considered before making generalization on these findings. FINANCIAL INTEREST: NONE
