Abstract Details

Purpose:

Reproducibility of subjective refraction in pseudophakic eyes and the comparison with autorefraction and aberrometry measurements.

Setting:

Vienna Institute for Research in Ocular Surgery, Department of Ophthalmology, Hanusch Hospital, Vienna

Methods:

This prospective study included patients who underwent cataract surgery. Baseline measurements were performed at least 8 weeks after cataract surgery. Subjective refraction was performed using the Jackson cross cylinder method by two different examiners, followed by five consecutive measurements with two different autorefractors (Topcon KR 8800, Japan and NIDEK Autorefractor ARK-560 A, Japan) and a Hartmann-Shack (HS) aberroemtry device (WASCA, Carl Zeiss Meditec AG, Germany). All measurements were repeated 1 week after baseline. Analysis was performed using the b-vector (blurring strength vector) which is a 3-dimensional vector that represents the sum of J0, J45 and M (=spherical equivalent).

Results:

In total, 100 pseudophakic eyes of 100 patients were included. The interchangeability of the spherical equivalent showed significant differences between the subjective refraction and autorefraction and between subjective refraction and HS aberrommetry device. We found considerably large values for astigmatism difference vector (AdV), as for SE, HS showed the highest deviation compared to subjective refraction. A good reproducibility was shown for all measurements – the best for autorefraction.

Conclusions:

Deviation between different methods to assess manifest refraction can be considered as clinically relevant. The relatively poor interchangeability of SE and astigmatism is a problem for optimizing constants for IOLs for power calculation. FINANCIAL INTEREST: NONE
