Abstract Details
Purpose:
The purpose of this paper is to present a case of episcleral arteriovenous malformation near limbus, the management, the follow-up and the discussion.Arteriovenous malformation is a continuity between an artery and a vein, without an intervening capillary bed.It can occur in several areas of a body as a congenital or acquired lession. Orbital AVMs are rare lessions, who can present with symptoms and signs of proptosis, limbal chimosis, restricted extraocular movements, secondary glaucoma and dilatation of the veins of the retina, conjuctiva and eyelids. Differential diagnosis includes intraocular tumours and pseudotumors.Especially, dilated limbal vessels tend to occur in malignant cilliary body tumors.
Setting:
General Hospital of Serres,Greece
Methods:
A 45-year-old female was referred to our institution, complained about abnormal symptoms in the right eye associated with itch and discomfort during the last 3 years. Her best corrected visual acuity was 20/20 in both eyes.On slit-lamp examination , abnormal, dilated vessels(AVMs) were noticed on the right temporal episclera at ciliary body area near limbus. There was no periocular edema,pulsation/bruit or proptosis. The patient had no history of orbital trauma or family history of vascular disorders.Ocular movements were not restricted and intraocular pressure was normal.Futhermore, fundus examination revealed that retinal vessels and optic nerve were also normal.
Results:
Every six months slit-lamp examination, gonioscopy and transillumination are performed. This frequent follow-up proves that no significant complications are occurred in the eye.Discomfort of the eye was treated using artificial tears.
Conclusions:
In our case report, episcleral arteriovenous malformation appears to be a benign stationary condition, that has no apparent associations and complications. However, orbital AVMs are vascular lessions with great clinical significance.Their management depends on patient-specific features.
