Age-related open-angle glaucoma: when cataract surgery is enough

Abstract Details

Purpose:

Phacoemulsification in patients with open-angle glaucoma is well known strategy. It allows to decrease IOP in the early period after operation on 3-5mmHg. In some patients it is enough for long period, but usually glaucoma still develop and we need to use some medications or anti glaucoma operation. Ophthalmic surgeons need some definite criteria for glaucoma patients associated with cataract - when Phaco is enough and when we need combined surgery.

Setting:

In Clinic 'Visiobud' in Kyiv we analyzed data of patients with age-related open-angle glaucoma associated with cataract before and after phacoemulsification. Our work can help ophthalmic surgeons in operation technique choice - only Phaco or combined Phaco with glaucoma.

Methods:

We assessed data of 114 patients with age-related open-angle glaucoma patients associated with cataract. We operated them by Phaco in 2013-2014. All patient were under our control during the next three years. Their glaucoma examination included IOP measurement, visual field assessment and OST of optic nerve. Criterium of effectiveness of treatment was IOP lower then 22mmHg, no medications, stability of visual functions and of optic nerve condition.

Results:

We had got decrease of IOP in all patient on 2-9mmHg after operation during first 6 month. But only 88 patients were not need to use any additive methods for glaucoma compensation. Before operation all these patient had big lens size (more then 4,8mm) and narrow anterior chamber (less then 3,0mm). They had early stages of glaucoma and short term of anti glaucoma drops use.

Conclusions:

For good glaucoma compensation it is possible to use only Phaco in patients with age-related open-angle glaucoma associated with cataract. But definite criteria are early stage of glaucoma and changes of lens size.
