Disadvantage of patching therapy in anisohypermetropic amblyopia

Abstract Details

Purpose:

The aim of this study is to evaluate the side effects of patching of non-amblyopic eyes for treating amblyopia and axial length (AL) of non-amblyopic eyes and their correlation in aniso-hypermetropic amblyopic.

Setting:

This cross sectional study has been conducted at Shahid Beheshti University of Medical Sciences.

Methods:

In this study 87 aniso-hypermetropic amblyopic cases with mean age of 8.11±2.17year, without strabismus and organic pathology were included. In terms of amblyopic treatment, cases were divided into three groups. In the first group, children have been treated merely with correcting glasses; in second group along with correcting glasses part time patching (4 hours or less) and home-practice were applied and in third group along with correcting glasses, full time patching (8 hours or more) and home-practice were applied. For both second and third study groups, non-amblyopic eye (the eye was emmetropia or only hypermetropia ≤+2.25 or compound hypermetropic astigmatism with spherical equivalent ≤+2.25 and best correcting visual acuity (BCVA)=20/20) had been patched with occlusion. The complimentary examination in all groups were assessing axial length (AL) of non-amblyopic eye, BCVA, and cycloplegic refraction.

Results:

All treated subjects were 15 children in first group, 57 children in the second group and 15 children in the third group. A statistically significant difference found between three groups in mean AL of non-amblyopic eye (p<0.001), the mean AL in the third group was more than the second group and in second group was higher than the first one. A statistically positive significant correlation found between the third group and first group in mean AL non-amblyopic eyes (r= 0.612. p<0.001, 2-tailed). It has been found a positive linear relation with r2=0.257, between third and second study groups in mean AL of non-amblyopic eyes; a positive linear relation with r2=0.407 between third and first groups in mean AL of non-amblyopic eyes.

Conclusions:

In aniso-hypermetropic amblyopic children with patch therapy, depending on the treatment period and the amount of eye closure, the AL of occluded eye increases, which eventually will increase the anisoaxial (the focal length difference between the two eyes). Therefore, we would suggest that in those subjects, instead of patching, other types of occlusion therapies or binocular therapies should be applied.
