Ab-interno canal surgery: step-by-step approach for beginners

Abstract Details

Purpose:

The purpose of this presentation is to present a step based approach on how to initiate Ab-interno canal surgery.

Setting:

Private practice. Instituto de Oftalmologia Tadeu Cvintal, Sao Paulo, Brazil.

Methods:

A series of Ab-interno canal surgery (Gonioscopy Assisted Transluminal Trabeculotomy – GATT- surgery, Goniotomy assisted by the Kahook Dual Blade and Ab-Interno canaloplasty) cases were filmed and photographed. The surgeon learning curve was grouped in topics and presented to help beginners on canal surgery.

Results:

This presentation will focus on a practical/graphic 10 step approach on how a general surgeon can initiate a canal based surgery: (1) Pre-operative preparation (2) Importance of visualization. (3) Importance of the anatomy (4) Microscopes tilt differences. (5) Microscope Optic quality. (6) How to identify the angle. (7) Incisions. (8) OVD used. (9) Tips and pitfalls. (10) How to initiate?.

Conclusions:

Ab-Interno Canal surgery, also known as minimally invasive Glaucoma Surgery, has evolved as one definite glaucoma treatment option. Initiating a new technique can be a challenge and few steps should be accounted for, especially: visualization, anatomic knowledge and technique.
