Comparison of central corneal thickness and endothelial cell density in patients with various types of glaucoma and patients without glaucoma

Abstract Details

Purpose:

The aim of this study was to assess the Central corneal thickness (CCT) and Endothelial cell density (ECD) and compare the difference amongst different types of glaucoma patients and also with normal individuals. This study was to estimate the CCT among different glaucoma subgroups indicating importance of corrected IOP measurement and thus make correct diagnosis and plan treatment. Also, to determine the changes in the corneal endothelial count seen in different types of glaucoma and to take certain precautions among these individuals while planning any anterior segment surgeries or procedures.

Setting:

Case control study conducted on Indian patient population presenting to the Outpatient department of Shekar eye super speciality hospital, Bangalore, Karnataka,India.

Methods:

Central corneal thickness (CCT) and endothelial cell density (ECD) was evaluated in 156 phakic patients between 40-80 years with specular microscope using Tomey EM 3000 Specular microscope. 77 patients of Glaucoma (36 eyes of POAG,11 eyes of PACG,14 of NTG,8 of ocular hypertension (OHTN) and 8 of PXG) were compared with age matched normal individuals. Right eye was taken by convention. CCT and ECD were compared between the controls and glaucoma and in between the glaucoma subgroups. All patients with corneal pathology, active infections, previous contact lens use or surgeries were excluded.

Results:

CCT in the Glaucoma cases was 513.70±36.35 and in controls was 520.75±34.14. CCT amongst glaucoma subgroups was seen to be thinnest amongst NTG 497.64±33.08μm and maximum in OHTN patients 531.25±23.88μm. Max CCT Difference was seen between NTG and OHTN subgroups(33.607μm) also associated with a significant difference between the CDR of two subgroups. The difference of CCT amongst ACG vs NTG(26.630μm) and between OHTN vs PXG(24.375μm) was seen.54.5% glaucoma cases had mean ECD of 2481.75±341.43. Patients with PXG had least ECD 2341.88±161.14, patients with OHTN had the max ECD of 2559.75±268.21. Maximum difference between ECD when comparing Glaucoma cases with controls was observed between PXG and Controls(191.29cells). On comparing the subgroups difference between ECD of PXG with OHTN(217.875cells) and PXG with OAG(167.319 cells) was noted the maximum. Difference between other subgroups of glaucoma was not significant

Conclusions:

This study concludes that NTG patients had the thinnest cornea whereas OCTN patients had the thickest cornea. Comparable ECD was seen in between different glaucoma subgroups and controls. However least ECD was seen in PXG and maximum ECD in OHTN patients. The thinner corneas seen in NTG patients and PXG patients having the least ECD thus indicate special precautions to be taken while planning anterior segment and cataract surgery in PXG patients and IOP adjustments in NTG and OHTN patients.
