Ophthalmic manifestation in patients with leukemia among King Abdulaziz Medical City, Jeddah

Abstract Details

Purpose:

Leukemia is a malignant disorder of hematopoietic bone marrow stem cells characterized by an abnormal over-crowding proliferation of leukocytes, which can be categorized according to the kind of predominant cells involved as myeloid leukemia or lymphoid and based on the period of the disease as acute or chronic leukemia. Some of which have a fast clinical course that could lead to a lot of extramedullary complications in different organs in the body such as meninges , testicles and orbits respectively that could be fatal or life limiting if not taken care of. To investigate the prevalence of ophthalmic manifestations in patients with acute and chronic leukemia both myeloid and lymphocytic presented at King Abdul-Aziz medical city in Jeddah, KSA to determine whether ocular and orbital manifestations are important in predicting a better prognosis for leukemic patients future-wise.

Setting:

King Abdulaziz Medical City, Jeddah, Saudi Arabia.

Methods:

This cross sectional descriptive, observational retrospective chart review study was conducted from all the medical records of leukemic patients who presented to king Abdul-Aziz medical city, Jeddah, during the last 4 years starting from January 2014 to June 2017. All leukemic patients who had a file at KAMC both adult and pediatric patients were included.

Results:

A total of 329 medical records of patients diagnosed with leukemia were reviewed. (61.4%) were males and (38.6%) females, the majority of them were of pediatric age 60.8%. Most of which were reported to have acute leukemia (81.5%). More than half of them were found to have normal cytogenetics (65.3%). And only (23.7%) patients were found to have ocular manifestations. Ophthalmic manifestations were more frequent in patients diagnosed with acute leukemia (25%). And the most common complaint was altered vision.

Conclusions:

This study revealed the reason for leukemic patient’s presentation to ophthalmology clinics was due to their complaints of altered vision, which was more common in acute leukemia than chronic and Most of them were seen in the pediatric age group. Therefore, it is recommended that all leukemic patients undergo regular ophthalmic examination, to enable the detection of early signs of CNS involvement or relapse of the disease.
