Purpose:
Description of clinical cases of corneal perforation in two pregnant women
Setting:
JSC Kazakh Eye Research Institute, Almaty, Kazakhstan
Methods:
Case report. Two pregnant women with perforation of the cornea were under observation. In the first case, the patient is 21 years old, the first pregnancy. Within 32 weeks, she received laser-coagulation of the retinal thinning zones on the right eye, concomitant diagnosis of OU – medium degree myopia, complicated by peripheral chorioretinal dystrophy. The second patient is 23 years old, the first pregnancy, 35 weeks, OU is a medium degree myopia. She had a history of the recurrent herpetic keratitis within the last 10 years, a local ophthalmologist was prescribed instillations of a corticosteroid in connection with another inflammation.
Results:
1. After laser coagulation of the retina the patient used diclofenac, prescribed by the doctor in the form of instillations for 3-4 days – for 1 month, contrary to the prescription of the doctor. At admission to the clinic, there was local perforation of the cornea. Urgent penetrating keratoplasty was made.2. At the time of admission to the clinic, the patient had a perforation of the cornea, about 2,5 mm in diameter. The lack of donor material in the country at that time, was not allowed to produce the keratoplasty. Ophthalmologists were forced to observe the patient for 4 months using contact lenses.
Conclusions:
It is necessary to remember the change in the biomechanical properties of the cornea during pregnancy. Strict monitoring of the use of local corticosteroids and NSAID s in pregnant women is necessary.
