Role of anterior segment optical coherence tomography in the evaluation of microbial keratitis

Abstract Details

Purpose:

1.To assess the role of anterior segment OCT (AS-OCT) by doing serial measurements of corneal thickness (CT) and infiltrate thickness (IT) in resolving microbial keratitis, can aid objective evaluation and response to treatment. and to assess whether retro-corneal features of inflammation can be imaged.

Setting:

The study was done at department of Ophthalmology, MLN Medical College, Allahabad,India

Methods:

This was a prospective study of 17 patients of microbial keratitis. The patients underwent slit lamp and AS-OCT examination on presentation and 3 follow up visits.

Results:

The mean CT in the infiltrated area and mean IT, both were largest at presentation (710.88 um and 288.76 um respectively). On follow up visits, mean CT and IT decreased. CT showed the most rapid decrease during late phase whereas IT decreased rapidly in the middle and late phase of treatment. The overall decrease in CT and IT over 14 days was statistically significant at 1.45% and 2.15% respectively.The reduction in CT could be due to reduction of corneal edema and that of IT due to resolution of inflammatory cells in resolving microbial keratitis. Characteristic morphological patterns seen were stromal edema, cystic spaces, thinning, scarring, epithelial defect, keratic precipitates and descemetocele.

Conclusions:

AS-OCT can show morphological characteristics of microbial keratitis and give a quantitative assessment at every follow up.
