Effect of single instillation of three different topical lubricants on tear film thickness in patients with dry eye disease

Purpose:
To test the influence of three different topical lubricants on tear film thickness (TFT) as measured with ultrahigh-resolution optical coherence tomography (UHR-OCT) in patients with moderate to severe dry eye disease (DED). In addition, the effect of single instillation on other signs and symptoms for DED was assessed.
Setting:
Sixty patients with moderate to severe DED were included in this randomized, monocenter, single masked, observer blinded parallel group study performed at the Department of Clinical Pharmacology, Medical University of Vienna.
Methods:
Patients were randomized to either receive either eye drops containing unpreserved sodium hyaluronate, triglycerides and phospholipids (DM05), eye drops containing unpreserved sodium hyaluronate (Vismed multi) or unpreserved sodium chloride eye drops (Hydrabak). TFT was assessed before and at defined time points after single instillation. In addition, standard tests for DED such as Ocular Surface Index (OSDI), a visual analogue scale (VAS), corneal fluorescein staining and determination of break up time (BUT) were performed before and after instillation.
Results:
The time course in TFT was significantly different between the three groups (p<0.001). Single administration of DM05 increased TFT over 40 minutes, while an increase in TFT over 20 minutes was observed after instillation of Vismed multi. Hydrabak had no effect on TFT. A significant improvement in VAS score vs. baseline was found after single instillation of DM05 (p=0.03) and Vismed multi (p<0.01), but not with Hydrabak (p=0.08). No differences in the other parameters were found between groups.
Conclusions:
In conclusion, single administration of DM05 and Vismed multi significantly increased TFT showing a longer residence time and improved subjective symptoms which is quite unusual after one single drop, in patients with moderate to severe DED, while instillation of Hydrabak had no effect on these parameters. Long term studies with multiple instillation times will be necessary to further investigate the long time effect of DM05 and Vismed multi on TFT and on clinical outcome variables.
