Purpose:
To evaluate the outcomes of combined penetrating keratoplasty (PKP) with scleral tunnel scleral-sutured posterior chamber intraocular lens implantation
Setting:
Department of Ophthalmology, Haydarpasa Traınıng and Research Hospital, Turkey
Methods:
Medical records from patients with aphakic bullous keratopathy who underwent combined PKP and scleral tunnel scleral-sutured PC-IOL implantation were retrospectively reviewed for preoperative indications and postoperative results of 27 consecutive patients (27 eyes) a 3 year peryod. Main outcomes measures were graft survival rate, visual acuity, and intraoperative and postoperative complications.
Results:
The principal indication for PKP was aphakic corneal edema. Mean postoperative follow-up was 23.9 ± 0.9 months (range 6-38 months). The best-corrected visual acuity improved at least 1 line on the Snellen chart in 25 eyes (92.5%) of the patients, with 44.1% of the eyes achieving acuities of 20/100 or better. Intraoperative complications included hyfema 2 eyes , vitreus hemorrhage 1 eye, suprachoridal hemorrhage 1 eye. Postoperative complications included new glaucoma in 5 eyes (18.5%), sutur exposure in 3 eyes (11.1%), graft failure in 3 eyes (11.1%) .
Conclusions:
PKP combined with scelral tunnel scleral-sutured IOL implantation can achieve acceptable outcomes with a low rate of intraocular lens dislocation. We reported the results using a scleral tunnel technique that allows scleral fixation of IOL in surgery prosedure closed systems with keratoplasty.
