Purpose:
To report the incidence and risk factors of aphacik glaucoma following infantile cataract surgery in the first year of life.
Setting:
Department of Ophthalmology, Haydarpasa Training and Research Hospital, Turkey
Methods:
A retrospective review of who had lensectomy for congenital cataract during the first year of life and were diagnosed with glucoma until sekonder IOL implantation surgery between 2011 and 2015. All patients had operated cataract extraction combined with posterior capsulorhexis and anterior vitrectomy. Preoperative; corneal diameter, axial length and cataract types were recorded. Glaucoma diagnosis was made according to the increase of the intraocular pressure of 21 mmHg, the enlargement of the corneal diameter, the increase of the axial length, the increase of the optical disc cup/disc ratio
Results:
Two hundred fifty three eyes met inclusion criteria. Twenty-six of the cataracts were 26 total, 182 nuclear, 12 lamellar, 11 posterior polar, 3 persistent fetal vasculature, and 21 was not reported. The patients underwent cataract surgery at a median age of 6.3±3.7 week (range, 6 to 50 weeks). Eyes of which 58 eyes (23%) developed open-angle glaucoma during a mean follow-up of 7.4 ± 2.8 months. Seventeen patients had bilateral glaucoma. All of the eyes diagnosed with glaucoma were initially treated with glaucoma medications. Thirty five eyes with glaucoma (70.1%) required at least one surgical intervention
Conclusions:
The incidence of aphakic glaucoma following infantile congenital cataract surgery is 23%. A significant proportion of patients required surgical intervention to control IOP. Age younger than 6 months at cataract diagnosis or cataract extraction, low axial lenght and short corneal diameter were the risk factors found to have statistical significance for the development of glaucoma
