Ocular dominance in lens surgery with multifocal lens implantation

Purpose:

Ocular dominance, sometimes called eye preference is the tendency to prefer visual input from one eye to the other. Approximately two-thirds of the population is right-eye dominant and one-third, left-eye dominant. The ocular dominance is very important in lens surgery. The aim of the study is to illustrate the importance of ocular dominance in lens surgery, clear or opacified lens, from our experience

Setting:

Ofta Total Clinic Sibiu Romania *, Faculthy of Medicine, Universithy Lucian Blaga Sibiu, Romania**, Ocular Surface Research Center Sibiu Romania ***

Methods:

We included in the study a number of 649 lens surgery, clear lens and opacified lens with the BCVA better than 0,1 in which we could determine the dominant eye (DE), in the OFTA TOTAL Clinc, Sibiu, Romania, between 2015 and 2016. There were 237 (36.6%) emmetropic eyes, 106 (16.3%) myopic and 306 (47.1%) hyperopic eyes. We used the Porta test for determining the DE. Phacoemulsification was performed with Centurion machine and, in 50 cases we also performed FLACS. We conducted the biometry using the Aladdin machine with indicated formulas for emmetropic eyes, myopic or hyperopic eyes. Of the total number of cases, 649, we implanted 573 monofocal IOL and 76 multifocal IOL (11,7%), Restor in 36 eyes, Panoptix in 37 eyes and other in 3 eyes; 14 eyes were myopic, 22 emmetropic and 40 hyperopic. Patients were operated on both eyes. We prepared the dominant eye for the distance vision and the non-dominant eye (NDE) for near vision. For DE we aimed at targeting emetropia and for NDE, small myopia. We tested the visual acuity (VA), binocular vision (BV) and the level of satisfaction of the patients through a questionnaire with 10 questions about distance and near vision in the usual daily activities

Results:

VA was very good, both for the distance and near vision. The BV was preserved. The satisfaction level of our patients was very good when we observe the ocular dominance. Most satisfied were the hyperopic patients followed by the emmetropic patients and the myopic patients. It is important to perform correct biometry with the best formula for every type of refraction. Monovision can be a good choice for patients’ satisfaction, especially for the myopic patients

Conclusions:

The multifocal lens implantation was of 11,7%. Ocular dominance is very important in lens surgery. The refactive status before surgery is also very important. The most satisfied patients were the hyperopic patients, followed by the emmetropic ones. Myopic patients prefer to have a good near vision, therefore monovision can be a good choice.
