Abstract Details
Purpose:
To establish the principle and provide illustrations and implementation of a method to simultaneously optimize the choice of the sphere and toric components of an IOL in the context of refractive cataract surgery.
Setting:
Private Clinic and Hospital in Champaign, Illinois, USA.
Methods:
A methodology of simultaneous optimization of sphere and toric IOL power calculation is presented where a global blur function characterizes the combined effect of residual sphere and residual astigmatism. An algorithm to minimize this measure is proposed as an alternative to the usual sequential approach that constitutes a 'greedy algorithm' and may result in less optimal choices of IOL. A specific implementation of the proposed optimal algorithm is presented in the context of the UniversIOL universal calculator and illustrated on 10 eyes undergoing cataract surgery.
Results:
The implementation of the algorithm demonstrates its effectiveness and high efficiency and the choice of the IOL in the 10 illustrative cases is shown to be optimal by establishing a full ranking of all IOLs selected from a universal IOL database.
Conclusions:
A methodology for the simultaneous selection of sphere and toric components of an IOL and its implementation in the UniversIOL universal calculator are demonstrated and shown to constitute a powerful tool for improved outcomes in refractive cataract surgery.
