Can a scoring system for assessing the risk of intraoperative complications be applied without modifications to every study population? -a pseudoexfoliation example

Abstract Details

Purpose:

To assess whether pseudoexfoliation alone or in correlation with other risk factors is associated with intraoperative cataract complications

Setting:

2nd Ophthalmology Department, Aristotle University of Thessaloniki, Thessaloniki, Greece

Methods:

In a retrospective cohort study, medical records of 1502 patients undergone phacoemulsification surgery were reviewed and exclusion criteria were applied. In the time of interest (01/01/2014-31/12/2015), patients were assessed preoperatively and all qualitative and quantitative characteristics of the eligible patients were gathered and processed according to our stratification system.

Results:

In overall 1792 eyes, pseudoexfoliation was present in 12.7% (227 eyes). In a multivariable regression analysis pseudoexfoliation had an Odds Ratio of 0.48 (p=0,022), appearing as a protective factor. Correlations between dense cataracts, poor pupillary dilation, zonular weakness, age, gender and open-angle glaucoma were made and the level of statistical significance was set at 0,05.

Conclusions:

The geographic distribution of PEX is favoring surgeons to familiarize and master the techniques needed to prevent complications in the existence of such factor. A modification to the stratification system used should include the reduction of the risk assessment at least for the regions of wide distribution of the factor.
