The analysis of the effectiveness of IOL pneumocompression method during phacoemulsification in prophylaxis of development of lens posterior capsule opacification

Abstract Details

Purpose:

We have earlier proposed the method of IOL pneumocompression at phacoemulsification (PE), which allowed achieving full contact of IOL optical part to posterior capsule (PC) in 93% of operated eyes in early postoperative period. Therefore, the purpose of our work was the evaluation of effectiveness of IOL pneumocompression method at PE of senile cataract for prophylaxis of development of lens posterior capsule opacification.

Setting:

State Institution Eye Microsurgery Complex named after academician S.N. Fyodorov, Khabarovsk, Russia

Methods:

124 patients (124 pseudophakic eyes), who underwent senile cataract surgery. All examined eyes were divided into 2 groups, depending upon the technical peculiarities of final steps of operation. Standard PE was performed in 63 eyes (1-st group). In 61 eyes (2-nd group), we injected air until the full contact of bubble with optical part of IOL was achieved, then IOL was pressed down to PC using spatula. “IOL – PC” interface was examined on 1-st day and 1 year postoperatively using biomicroscopy and Scheimpflug analysis methods. The structure and transparency of PC was investigated biomicroscopically 1 year post operation.

Results:

All operations passed uneventfully. Postoperative period was unremarkable in all cases. There were cavities of different size between IOL and PC and folds of the latter at 1-st postop day in 55 eyes (87,3%) of patients of the first group. On the opposite, optimal interface was achieved in 56 eyes (91,8%) of the second examination group. One year later 45 eyes (71,5%) of the first group had epithelial lens cells in the optical part of PC. At the same time, 50 eyes (82%) of patients of the second group were characterized by the absence of PCO in the central zone.

Conclusions:

The conducted research showed high effectiveness of IOL pneumocompression method, which allowed preserving the transparency of optical zone of posterior capsule in 82% of pseudophakic eyes. Besides using IOL pneumocompression method, it is necessary to provide qualitative removal of viscoelastic from the IOL posterior surface and from posterior capsule for prevention of PCO development.
