In vivo histomorphological structure of the cornea and corneal flap characteristics after LASIK procedure using two different femtosecond laser platforms

Abstract Details

Purpose:

To compare in vivo histomorphological structure of the cornea, corneal flap predictability, visual and refractive outcomes after LASIK using Femto Visum (Russia) and Femto LDV Z6 (Switzerland) femtosecond laser (FSL) platforms in myopia treatment

Setting:

S. Fyodorov Eye Microsurgery Federal State Institution, Moscow, Russia

Methods:

The study included 192 patients (192 eyes) with myopia. Corneal flaps were created with Femto Visum (Visum group,n=98, included Visum subgroup,n=36) and Femto LDV Z6 (LDV group,n=94, included LDV subgroup,n=34) FSL platforms. Excimer laser ablation was performed with Microscan Visum (Russia) platform. At 1 week, 1, 3, 6 and 12 month after LASIK in vivo histomorphological structure of the cornea was examined with Confoscan4 in Visum and LDV subgroups. At 1 month after LASIK the corneal flap thickness was measured in the horizontal and vertical meridian using optical coherence tomography, in addition flap diameter was measured in the horizontal meridian

Results:

One year postoperatively, the mean SE decreased in the Visum group from -3,73±1,42D to -0,07±0,24D (p<0.001), in the LDV group from -3,81±1,44D to -0,08±0,23D (p<0.001) (comparison between the both groups p>0.05). The main in vivo histomorphological changes in the cornea occurred in the anterior and middle layers of the corneal stroma during post-operative period in both subgroups (comparison between the both subgroups p>0.05).The underlying corneal structures remained intact. The mean deviation between achieved and attempted flap thickness and diameter had no statistically significant difference between the both groups (p>0.05). Flaps morphology was uniform in the Visum and LDV groups (p>0.05)

Conclusions:

LASIK procedures using Femto Visum and Femto LDV Z6 platforms lead to similar histomorphological changes of the cornea, characteristics of the corneal flap, visual and refractive outcomes in post-operative period (p>0,05)
