Preoperative near visual acuity as a predictor for postoperative distance visual acuity in patients with cataracts

Abstract Details

Purpose:

The purpose of this study is to assess whether preoparative near visual acuity can be used as a predictor for postoperative distance visual acuity for patients with moderate to severe cataract (excluding mature cataract).

Setting:

Neega Eye Center

Methods:

This is a prospective study. Two hundred eighty seven patients with cataract with grade III and grade IV were evaluated preoperatively for distance and near visual acuity. Mean age of the patients was 65. At postoperative month one, best-corrected distance visual acuity was also obtained. The postoperative best-corrected visual acuity was compared to preoperative near visual acuity. The near visual acuity test was performed by having the patient hold a near card at 30 cm in front of his/her face and shining a bright focused light on the near card letters.

Results:

Mean preoperative near visual acuity was 0.72 while mean postoperative best-operative visual acuity was 0.64 (P= 0.001). There was no statically significant difference between the preoperative near acuity and postoperative best-corrected visual acuity at month one.

Conclusions:

Preoperative near visual acuity can be used as a quick and practical tool to tell the postoperative outcome after cataract surgery in those patients where the density of the cataract preclude clear view to the fundus. It does not work when the cataract is mature or hyper mature because the patient is not able to see the near card letters. When 20/20 vision is not obtained on near card visual acuity, this indicates the presence of other pathologies other than cataract. It also predicts how much best-corrected distance visual acuity the patient will obtain after cataract surgery in the presence of other pathologies other than cataract.
